Client Consent Forms
 

At Solace and Sanctuary, your safety, comfort, and understanding are our top priorities. Our Consent Forms ensure that you and/or your child are fully informed about how counselling services are delivered and how personal and therapeutic information is managed. These forms support ethical practice and comply with Australian Counselling Association (ACA) standards and Australian privacy legislation.

Please note: These forms are usually completed at your first session or during ongoing sessions as appropriate.
 

Why Consent Forms Matter

Consent forms:

  • Ensure clarity and transparency about counselling sessions and optional supports

  • Maintain safety and comfort, including the use of sensory tools and food/beverages

  • Document agreement in line with ACA ethical guidelines and Australian privacy law

  • Give clients or guardians autonomy to make informed choices
     

Our Key Consent Forms

1. Client Consent for Record Storage and Management (Halaxy)

This form explains how your personal and therapeutic information is stored securely using Halaxy, a professional clinical records platform.

It covers:

  • Types of information collected (contact details, session notes, invoices, consent forms)

  • Confidentiality and who may access records

  • Legal requirements for sharing

  • Record retention and secure deletion

Why It Matters:
Secure records ensure ethical, safe, and effective therapy while complying with ACA, PACFA, and Australian privacy standards.

When Signed: At your first session.
 

2. Permission to Offer Sensory Regulation Tools in Counselling Sessions

This form allows clients or guardians to consent to the use of sensory tools during sessions. These tools support:

  • Emotional regulation and grounding

  • Comfort and engagement during therapy

  • Development of coping strategies

Features:

  • Offered only with the client’s assent

  • Optional and adjustable during sessions

  • Provided at no charge, including small items to build a sensory tool kit for use in sessions and at home

Why It Matters:
For children and neurodiverse clients, sensory tools enhance engagement and reinforce therapeutic skills.

When Signed: At the first session or as sensory supports are introduced.
 

3. Parental/Guardian Consent for Food or Beverages During Counselling Sessions

This form allows guardians to consent to light snacks or drinks offered during sessions to:

  • Promote comfort and wellbeing

  • Support focus and engagement

  • Foster a welcoming environment

Features:

  • Guardians provide information about allergies, dietary restrictions, or other health concerns

  • Participation is voluntary and consent can be withdrawn at any time

Why It Matters:
Ensures food is offered safely and responsibly while maintaining session focus and wellbeing.

When Signed: At the first session or when food/beverages are introduced.
